Please arrive on time.  This is your time and you will be charged for the time scheduled.  A patient more than 15 minutes late may not be seen unless we have been notified in advance of your late arrival.

 

CANCELLATIONS & MISSED APPOINTMENTS

Please provide 24-hour notice of cancellation.  If you miss or cancel and appointment within 24-hours you will be charged for the appointment. Our policy seeks to respect the time constraints and integrity of all parties; if you miss an appointment three parties are affected: you do not get treated, we have an open block of time, and another person was not able to schedule an appointment in your time slot.

 

DISMISSAL OR DENIAL OF TREATMENT

Patients who show inappropriate conduct, non-or late payment of fees, or present safety concerns may be denied treatment.

 

FINANCIAL POLICY

Payment is due in full at time of service.  For your convenience we accept cash, check, or credit cards (Visa or MasterCard only).  Returned checks will be charged an additional $25 fee. Electronic receipts are available.

 

NUMBER & FREQUENCY OF TREATMENTS

The number and frequency of treatments needed depends on the nature, severity and duration of the condition. Many patients experience a certain amount of relief after just one or two treatments. Most conditions require an average of 6-12 treatments. Some will respond well within 4-6 visits and others may require a longer series.

 

INSURANCE POLICY

We are in-network with some United Health Care plans. You are responsible to verify your coverage specific to your insurance plan.  We will send in the required paperwork for in-network plans. For out-of-network patients, payment is due at time of service; we can provide the necessary codes & receipts for you to send with your claim for reimbursement. Your insurance customer service department can tell you about your plan. Ask, "Does my plan include out-of-network acupuncture benefits?  Is there a deductible that needs to be met first? Are there restrictions on conditions covered?"
